In aesthetic operations performed to correct the shape of the nose, two different methods are used as open and closed surgery. The procedures performed in both surgeries are the same and the only difference is that the open surgery is performed by cutting the skin between the nostrils, while the closed surgery is performed completely through the nose and therefore there is no incision scar (scar tissue) visible from the outside. Female and Male Nose Job Differences:

Rhinoplasty, which was an cosmetic procedures  preferred by women in the past, but nowadays become a highly preferred plastic surgery procedure by men today. Currently,in our plastic surgery clinic male and female ratio was almost equal to each other.

Desired features in men: Nose-lip angle should be less upturned than in women, a straight masculane nasal ridge. Otherwise, if the nasel bone is too curved and the tip of the nose is too much raised, a feminine (feminine) appearance appears.

What is Revision Rhinoplasty Surgery?

Revision rhinoplasty surgery is called revision rhinoplasty surgery, which is performed on patients who cannot achieve the desired result from previous rhinoplasty surgery and are not satisfied with the result.

Revision rhinoplasty is usually an operation that requires more surgical experience than the first nose surgery. In revision surgery, the surgeon has to deal with tissues that have not healed properly.

Adhesions from previous surgery make it difficult to separate the skin and soft tissue from the cartilage. Cartilage tissue may often be required to reshape the nose. These cartilages are usually supplied from the patient’s own rib cartilage or fresh frozen cadaver cartilage.

·       In which cases is revision rhinoplasty surgery necessary?

Following the popularisation of cosmetic rhinoplasty surgeries all over the world, the frequency of rhinoplasty surgeries performed for the second or more times has also increased. Revision nose surgery may sometimes be due to the previous operation not being successful, not achieving the desired results, and sometimes due to the natural healing process.

Revision rhinoplasty surgeries are divided into two as minor (Touch Up) or major (Blow = Detailed) depending on the scope of the problem.

Every rhinoplasty surgeon may have revision cases. In our clinic, our minor revision rates, also called retouching, are 3% and major revision rates are 1%. Generally, when retouching is required, we can produce solutions with simple touches under local (regional) anaesthesia. However, in rare cases, general anaesthesia and the use of rib cartilage may be required for major revision surgeries.

Normally major revision surgeries are more complex and costly. However, when it comes to our own revisions, it is sufficient for the patient to cover only the hospital fee and we do not charge a surgical fee.

·       Is it necessary to use cartilage in revision rhinoplasty?

In revision surgeries, cartilage and soft tissue are often needed. The reason for this is that the cartilage structures in the nose are usually removed in previous operations. Therefore, rib or in some cases ear cartilage can be used. In our clinic, rib cartilage is usually used. In addition, depending on the patient’s condition and consent, the use of cadaver cartilage, which we call allograft, is a very safe and comfortable application today.

·       When can revision nose surgery be performed after the first surgery?

Many nasal surgeons are of the opinion that revision rhinoplasty surgery can be performed 4-6 months after the first surgery. However, if reoperation is required after the second and more nose surgeries, which we call multiple revision, the waiting period is longer. In our own practice, we plan revision surgery at least 8-12 months after the first revision. However, if the person will have a 2nd revision rhinoplasty (i.e. 3rd nose surgery), it is recommended to wait at least 12 months for the tissues to settle.

The Rhinoplasty Procedure: Step-by-Step Guide

Depending on the developing technology and the experience of our Specialist Professor Dr Seçkin Ulusoy, the treatment process for rhinoplasty operations is easily completed without feeling pain.

In aesthetic applications, patient suitability is important for the operation to be successful. In addition to the detailed examination and simulation analysis to determine whether the patient is suitable or not, the specialist physician also decides by listening to the patient’s medical history.

• We will perform your discharge from the hospital the day after the operation (In special requests, we can be discharged 6 hours after the operation, but in our practice, we recommend our patients stay in the hospital for 1 night).

• You will have almost no pain from the operation period, but there will almost always be limited edema and bruising, especially the next day. After 3-4 days, edema and bruises will be relieved and you will be able to breathe easily with the removal of your silicone leaflet.

• You can visit every part of Istanbul from the 2nd day after the operation.

• Relatively expensive. Insurance companies often exclude payments, but some insurers can meet if structural needs are corrected for medical reasons such as breathing problems.

• Since there will be visible swelling and bruising for the first week, you will stay away from the active work environment for 5-10 days depending on your workplace environment.

Rhinoplasty Aftercare: Recovery, Healing, and Post-Operative Care

Depending on the condition of the thick skin, it may be necessary to give additional treatments before or after the surgery by consulting the Dermatology doctors. In cases with patients having thick skin, conducting liver and other blood tests allows for the implementation of Vitamin A treatment. This approach is often used and can be quite successful in refining the skin texture.

The amount of cartilage tissues left behind in men and women during aesthetic nose surgery varies. Post-operative bruising and swelling are more common in men, and the final state of the nose occurs more longer.

Recovery Process After Rhinoplasty:

Cosmetic nose surgeries are mostly performed under general anaesthesia, but local anaesthesia may be preferred for minor changes. Cosmetic rhinoplasty surgery takes an average of 3 hours and depending on the patient’s condition, the patient may be hospitalised for one night as a precautionary measure to make the first night more comfortable and safe. Some patients may be discharged after 4-6 hours after surgery upon their special request.

·       How is the first day after rhinoplasty?

Generally, it is sufficient to stay in your room in the hospital for the first day and night after rhinoplasty. Since you are under general anaesthesia, it will be better for your safety if you are followed by hospital staff overnight. In the meantime, you will be given antibiotics, antipyretic and anti-edema drugs intravenously. In addition, ice is applied under your eyes for 15 minutes every hour.

You will be discharged the next day after the operation if you are deemed suitable after observation by the surgeon. Before discharge, you will be informed about the care and cleaning of the operation area. You will be prescribed the medication you need to use and you will be informed in detail about the points you should pay attention to.

After the operation, you can usually drink water after 1 hour and eat after 2 hours. You should definitely call the ward nurse when you first stand up. Because you may feel slightly dizzy after general anaesthesia. If you feel comfortable, you can walk around the hospital freely, provided that your nurse is with you.

·       Is a tampon placed in the nose?

Cloth tampon application after nose surgery has been almost completely abandoned especially in our country. Thin plastic silicone applications into the nose to accelerate postoperative healing and reduce bleeding have now completely replaced the cloth tampon. Silicone tampons are thin and allow nasal breathing (Doyle splint) and are removed on the 3rd-6th day of surgery.

In addition, the application of plaster to shape the nose has been largely replaced by medical products called splints. These splints can be made of materials that soften in hot water and harden when cooled (thermal splint) or aluminium.  The splint is removed 5-6 days after the operation and a bandage is applied afterwards. When you remove the bandages yourself in the shower after one week, no additional shaping is required. After the bandages are removed, you can start the massage applications recommended to you.

·       What is the general process after the operation?

After rhinoplasty surgery, the patient is generally hospitalised in the hospital for observation overnight, depending on his/her health condition. The patient is allowed to walk within 1-3 hours after the operation.

If no health problems such as nausea and dizziness are observed after 1-3 hours, the patient can drink water and eat.

Depending on the general health status of the patient, the patient is discharged on the same day and in some cases the next day. During the recovery period, the patient is checked in the clinic on days 2-6. In the second control (6th day), nasal plasters and splints are removed. After that, it is possible to follow up online.

·       Does Edema, Bruising and Swelling Occur?

Aesthetic nose surgery is a surgery with low risk and complications. After rhinoplasty surgery, the patient is generally hosted in the hospital to stay under observation for one night, based on their general health and comfort status.

If a nausea health problem is not observed at the end of 1-3 hours, the patient can start drinking water and eating.

In some cases, the patient demands to be discharged home on the same day, but our suggestion is a night stay in the hospital, and the specialist doctor should come to the controls during the recovery period.

The patient, who has had a comfortable process without complications in the operation, can return to work life at the end of 5-7 days, which is the recovery period. But in regards to less swelling technologies, he or she can come back to the daily routine even on discharge day.

During and after this period, mild swelling and bruising are considered quite normal and disappear spontaneously in a short time. After 4-5 days, the person’s appearance can be covered with make-up and these swellings gradually decrease next couple of days. 2) Between 1-3 Months: Which Changes Are Seen?

1) Use of Cold Compress

• Applying a cold compress to the eye area in the first 48 hours after surgery can significantly reduce oedema and bruising.
• Cautions: The compress should be applied on the cheeks and around the eyes, not directly on the nose. Applying compress for 20 minutes and taking a break for 10 minutes is an ideal approach.

2) Keeping the Head High

• Keeping the head elevated while lying down (supporting it with two pillows or using a bed at a 45-degree angle) allows the oedema to decrease faster. Thanks to gravity, blood circulation is regulated and it is easier for the swelling to go down.
• Continuity: It is useful to maintain this position for the first 1-2 weeks.

3) Reducing Salt Consumption

• Avoiding salty foods helps reduce oedema. Excessive salt consumption can lead to fluid retention in the body, delaying the reduction of swelling.
• Recommendation: Drinking plenty of water and following a low-sodium diet promotes healing.

4) Avoiding Heavy Exercises

• It is important to avoid intense physical activity for the first 4-6 weeks. Exercise can increase blood pressure, trigger oedema and lead to permanent bruising.
• Alternative: Taking light walks improves blood circulation and does not harm the healing process.

5) Using Edema Reducing Support Products
