What Is the Adenoid?
The adenoid is lymphoid tissue located at the back of the nasal cavity, in the nasopharynx, and is part of the immune system. Adenoid enlargement, which is common during childhood, may lead to recurrent infections, nasal congestion, and snoring. It usually shrinks with age and may disappear in adults. The adenoid is an important member of the lymphoid tissue chain known as “Waldeyer’s ring,” which forms the first line of defense against microbes entering the body. This ring also includes the palatine tonsils that we all know, the lingual tonsils at the base of the tongue, and the small lymphoid tissues around the Eustachian tube that provides ventilation to our ears.
Every child is born with an adenoid. Especially during the first years of life, while the body encounters bacteria and viruses from the outside world, it actively works and grows to train the immune system. It usually reaches its largest size between the ages of 3 and 7. As the child grows and the immune system matures, the function of the adenoid gradually decreases and the tissue enters a natural shrinking process. During adolescence, it almost completely disappears in most children.
What Is the Function of the Adenoid in Our Body?
The most basic function of the adenoid is to serve as a kind of “training camp” for the body, especially during early childhood. It captures foreign visitors entering through the nose and mouth, namely bacteria and viruses, like a filter. After recognizing these microbes, it sends a message to the immune system, which is the body’s defense army, and triggers the production of “warrior cells.” This process enables the child’s immune system to recognize microbes, learn how to fight them, and create a “memory” for future encounters.
Because of this important function, the presence of the adenoid is critically important during the first years of life. However, as the child grows and the immune system develops stronger and more organized mechanisms to perform these tasks, the need for the adenoid decreases. Therefore, contrary to one of the greatest concerns of parents, removal of the adenoid when it causes problems does not lead to weakness in the child’s immune system. Other immune tissues in the body easily take over its function.
Why Does the Adenoid Enlarge and Cause Problems?
It is completely normal for the adenoid to be somewhat large in children. However, some factors may cause this tissue to enlarge excessively and become inflamed, leading to a number of problems. There is usually one or more triggers behind excessive enlargement of the adenoid. The most common of these are:

The diagnosis of adenoid enlargement requires a careful evaluation process. The process begins by listening to the family’s concerns and is confirmed with the help of modern technology.
The first step is always a detailed discussion with the parents. Issues such as what complaints the child has, how long these complaints have continued, and what their sleep is like are clarified. Then, a complete Ear, Nose, and Throat examination is performed. During this examination, clues such as the child’s general facial structure, whether they breathe through the mouth, and their manner of speaking are evaluated.
The gold standard method for confirming the diagnosis is nasal endoscopy. This procedure is performed by gently advancing a thin and flexible tube with a tiny camera at its tip through the nose. In this way, the adenoid tissue is seen clearly and enlarged on a screen. The physician can directly see the size of the adenoid, how much it blocks the airway, and whether it closes the openings of the Eustachian tubes connected to the ears. This procedure is completed within a few minutes under outpatient clinic conditions, does not involve radiation, and is extremely reliable. In some cases, especially in young children who have difficulty cooperating with the examination, a lateral neck X-ray may also provide an idea. If severe sleep apnea is suspected, an overnight sleep test (polysomnography) may be requested to confirm the diagnosis and document the severity of the condition.
Can Adenoid Enlargement Be Treated Without Surgery?
Yes, definitely. In fact, non-surgical methods are the first choice in many cases. The decision for surgery is always the last resort and should not be made hastily. Especially if the symptoms are mild or have recently started, there are effective medical treatment methods that are tried first and provide quite successful results. These approaches are:
- The “wait and see” approach
- Nasal sprays containing corticosteroids
- Antibiotic treatment (if there is an active bacterial infection)
- Allergy medications (antihistamines)
- Reflux treatment
The most effective among these methods is generally corticosteroid nasal sprays. These sprays act directly on the adenoid tissue, suppress inflammation, and help the lymphoid tissue shrink. However, it is most commonly performed between the ages of 3 and 7. The main conditions that lead to the decision for surgery are:

How and Through Where Is Adenoid Surgery Performed?
The biggest questions about adenoid surgery concern how the procedure is performed. The answer to the question “Is adenoid surgery performed through the nose?” is clear: No. The surgery is performed entirely through the mouth, and there is no incision, stitch, or scar outside the body.
The procedure is always performed under general anesthesia so that it is completely painless and comfortable for the child. During the surgery, the mouth is kept open with a special instrument and the nasopharyngeal area is reached. Today, different techniques are available for this surgery.
The traditional curettage technique is the oldest method, in which the surgeon scrapes the adenoid with a special spoon-like instrument while relying on the sense of touch. Are There Risks of Adenoid Surgery and Can the Adenoid Grow Back?
Adenoidectomy is one of the most commonly performed and safest surgeries in childhood. However, like every surgical procedure, it carries some risks, although very rarely. Bleeding is the leading risk, but with modern techniques, this risk is well below 1%. Risks related to anesthesia, infection, or temporary voice changes are other very rare possibilities.
The question “Can the adenoid grow back?” is one of the issues families are most curious about. Since the adenoid does not have a clearly defined capsule like the tonsils, it theoretically has the potential to regrow from microscopic tissue left behind. However, this situation is extremely rare, especially when the surgery is performed under endoscopic visualization with modern techniques such as microdebrider or coblation. This is because these techniques allow almost all of the tissue to be removed. Significant regrowth sufficient to cause symptoms and the need for a second surgery are extremely rare.
